To request a transcript, please complete the Transcript form and include a personal check or money order made payable to the University of Illinois for $5.00 for one transcript and $3.00 for each additional transcript. Transcript requests will be processed within 3 business days and returned by mail to the address(s) provided. Mail your request to:

Duplicate Certificates
To request a duplicate certificate, please complete the Certificate form and include a personal check or money order made payable to the University of Illinois for $3.00 for each replacement.
